I don't like the item itself so I opened mine.

Got the card and the flower, so I'm happy. I'll be able to sell the card and keep the flower for when I get around to actually decorating my neohome.

It is definitely not Premium only.

It seems to appear randomly.

It appears in the very first item spot, top left. I got mine after I refreshed a couple times.

It had:
A Mysterious Valentines Card and a Ruby Heart Necklace
